Join SCMA Practice Innovations and South Carolina Office of Rural Health on September 25, 2020 for a virtual clinical workshop:  Innovations in Patient-Centered Medical Home and Quality Improvement (The South Carolina Medical Association (SCMA) designates this Live Activity for a maximum of 1 AMA PRA Category 1 Credits™The workshop will feature an in-depth panel discussion where we will have an opportunity to hear from recipients of 2020’s SCMA and SCORH Advanced PCMH Initiative Award on best practices, quality improvement strategies, and data management protocols. Your webinar participation will be an excellent opportunity to develop new skills in quality improvement and learn from other practices who have implemented innovative patient-centered medical home processes.  Stay tuned for an announcement of applications for the 2021 Advanced PCMH Initiative which provides a grant to help practices achieve PCMH status.

Objectives:

1. Understand the strategies for developing effective patient partnerships, care planning, and team-based communication for overall patient engagement in primary care settings and with coordinated care facilities.

2. Recognize the processes of quality improvement and the importance of information systems in patient care, promoting workflow, and data collection and reporting in PCMH.

3. Discuss key considerations, actionable plans, and best practices to enhance PCMH among practice team and meaningful engagement with patients in the future.
